Spider-Man transformations question.

I have a small page devoted to Spider-Man for a role playing game, and on it I have a very basic history of the character.

One of the things I wanted to add were all the Spider-Man transformations that have taken place to the character over the years. You know what I mean, times he gained new powers, lost powers, became something else etc.

I know of most of them - cosmic powers, powerless, 6 arm, man-spider, spider-hulk, etc.

Does anyone have or could they make a list of which issue of what comic he changed, a brief description of how he changed, then in what issue he changed back and again how?

Tsk, tsk ... how can you have forgotten the Spider-Lizard? (SPECTACULAR # 39-40).

And then there was spider-carnage...sometime during the time when ben reilly was spider-man, i believe over in the early issues of sensational spider-man.

There was also Spider-Venom in one issue of Spider-Girl. For that matter didn't the black costume make Spidey stronger and his webs tougher. Then there was the short lived Spider-Armor.
