AFAIK, Carnage last appeared in Peter Parker, Spider-man 13. Venom had previously consumed Carnage's costume, leaving just Cassidy to appear in this issue. Started out as a good chance to maybe look into the mind of this once popular villain, but Mackie didn't seem to want to do any serious exploration of the character, so basically Spidey just shows up and kicks his butt real fast.

I don't know about the traditional versions, but Brian Michael Bendis has confirmed that Venom will appear in his Ultimate version this December (and, presumably, January, February, etc. until that story arc ends.)

BMB has said he doesn't like alien symbiotes. I wouldn't assume that Venom has anything to do with aliens or costumes at the moment.

There was one more appearance following that. One of the last issues of 'Webspinners' (13 or so) involved the revelation that Cletus still had a mental bond to the Carnage symbiote, which lead to him escaping prison and heading into the Negative Zone. That of course lead to Spidey taking on his 'Spider-Man Unlimited' costume, from the cartoon of the same name.
